Before Your First Visit
Our team will mail a health history and information form to your home when you call to schedule your first visit. Completing this form prior to your arrival facilitates beginning your first visit without delay. Please also feel free to look over the Privacy and Health Insurance Portability and Accountability Act (HIPAA) Information link here; this will also be given to you at your first visit. Additionally, if you have orthodontic insurance coverage, please bring your card so that we may assist you in filing for reimbursement from your insurance carrier. Your initial records and examination is complimentary!
FIRST VISIT
Thank you for choosing our office for your orthodontic evaluation. We want to assure you that your first visit will be highly informative, and we hope, quite enjoyable.
Upon your arrival, please say hello to our receptionist who will assist you during check-in. Dr. Henson will be notified that you have arrived and you, or you and your child, will be provided a tour of our office so that you may become familiar with the comfortable environment, latest technology, and cleanliness that our office strives to maintain.
At this first visit, our objective is to identify the long-term orthodontic needs for you or your child. A full set of orthodontic records including x-rays, molds of your teeth, and orthodontic photographs will be taken and you will receive a thorough orthodontic examination by Dr. Henson.
One of three outcomes will be determined:
• Treatment IS recommended now
• Treatment IS NOT recommended at all
• Treatment WILL BE NEEDED, but the timing of such will depend upon close monitoring of growth and development, tooth eruption, or other factors
We will review all findings in detail with you and discuss any questions or areas of concern you may have. Dr. Henson and his team enjoys spending time to review your case specifically, outlining your personalized treatment options which they will thoroughly explain in detail at your first visit or if necessary at a following consultation visit. Financing options are also discussed as we strive to make orthodontic treatment affordable and will tailor financing to meet your specific goals.
